Certain forms are required for clearance and the Athletic Director will check the student's grades

for the last reporting period. Student athletes must maintain a 2.0 GPA or higher,

and not have more than one "U" in Citizenship

or one "F" to be eligible to participate in a sport

Our Athletic Clearing process is online. Below is the link for the Athletic Physical Form.

Please take this form to your doctor for your physical.

For First Time Users: Go to www.homecampus.com and create an account

and then register your student for their sport.

For Returning Athletes: Log in to your Athletic Clearance Account

and start the clearance process for the new school year.

For Multiple Sports: On the last page at the very bottom

you can click on the other sports your student will participate in.

The completed and signed physical form must be scanned

onto the student information page in athletic clearance.

You will receive a confirmation sheet by email

once you have completed all the necessary information into Athletic Clearance.

Please sign and turn this into the ASB Office along with a copy of your completed physical.
